We provide primary care services as part of a fully integrated primary, community, mental health and acute hospital structure across North Wales, to roughly 689,000 residents.
The 7 health boards across Wales have created ‘primary care clusters’ – 64 groups of neighbouring GP practices and partner organisations which provide services for their local populations. There are 14 Primary Care Clusters within the North Wales area. East Area Cluster Team covers the counties of Flintshire and Wrexham; Central Area covers Conwy & Denbighshire, and West Area covers Gwynedd & Anglesey.
Within these clusters are a mix of both independent & Health Board managed practices.
